Required for invitation letter (if needed) and for clinical record. Spell exactly as on passport. Don't use nicknames or partial names — Korean clinical records require legal name.

Planned travel dates (range OK)

Specific date or range. Earliest possible / latest possible dates. JRYN finds available slots within your range. If your dates aren't flexible, share specific dates and we work to fit them.

Treatment interests or concerns

Either specific treatments ('Ultherapy Prime full face') or general concerns ('jawline sag', 'glow', 'dark spots'). JRYN translates concerns into recommended treatments. Both inputs work.

Photos showing relevant areas

Selfies in good lighting from front, left side, right side. Bare skin, no makeup if possible. Helps Dr. Lee evaluate before consultation. Saves clinic time and helps you get accurate quote.

Allergies and current medications

Especially: blood thinners (Aspirin, Coumadin, etc.), retinoids, recent fillers/botox, steroids, antibiotics. Pregnancy or breastfeeding status. Some treatments are contraindicated; sharing upfront prevents day-of cancellation.
